Dear Parents,

 

Within the next few days, you will be receiving via e-mail several forms that pertain to our upcoming registration.   (These forms can also be found on the SMM website under school office/registration forms.)  Within today’s information, you will receive the forms that must be brought with you to registration.  Please note: we will not have extra forms available in the office.

 

These forms are as follows:

Financial Document

Physical Education - Report of Injury, Illness and/or Operation (ONE PER FAMILY)

Internet and Electronic mail user agreement (ONE PER CHILD)

Health Screening Form (ONE PER CHILD)

2nd Grade Children Sacramental Preparation Form

4th Grade Children Related Arts Form

5th Grade Children Related Arts From

6th Grade Children Related Arts Form

8th Grade Children Sacramental Preparation From

 

Please remember the registration is as follows:

Dates:               June 9 – June 20, 2008

Times:              8:00am – 3:00pm Monday, Tuesday, Wednesday and Friday

                        8:00am – 8:00pm Thursday

 

The registration process will take place in the school office and appointments are not necessary.  If for some reason you cannot register during these designated times, please contact the school office to make other arrangements.  We encourage everyone to register by June 20, 2008, because after this time, you will be assessed a $100.00 late fee. 

 

Again, all the registration forms and money should be brought to the school office and not to the parish office.  Please do not forget to bring all the forms (including the optional forms that will be sent next week) with you that apply to your family because failure to do so may hold up your registration process.
