Please return this form with registration materials for the Winter 2008 no later than Feb. 19, 2008.  If you and your child are interested, please remember that class size is limited.

Payment of Fees

All fees are due by Tuesday, February 19, 2008.  Registration forms and checks will reserve a place according to the order of the forms and checks received.  Make checks payable to Saint Margaret Mary School.  Send your check in an envelope marked ASEP to the attention of Mrs. Martin.  Sorry, no make-up classes and no refunds.     Check enclosed for                   Ck. #                    . If paying for more than one student, staple forms together and write one check.
